The Georgia Pediatric Program helps eligible medically fragile children receive medically necessary care in the home and community.
Depending on the child's needs and authorization, GAPP may provide skilled nursing, personal care support, or both. Applications and authorization requests are coordinated through an enrolled GAPP provider agency.
Eligibility is based on the child, the medical need and Georgia Medicaid requirements.
Age
The member must be under age 21.
Medicaid
The child must meet applicable Georgia Medicaid eligibility requirements.
Medical Necessity
Care needs must be supported by physician orders and clinical documentation.
Authorization
GAPP services require Medicaid review and prior authorization.
Support built around the approved plan of care.
Legacy of Hope provides authorized services according to the child's individual medical needs, physician orders and approved plan.
Skilled Nursing
Professional in-home nursing for children who require skilled clinical care.
- Health monitoring and medically necessary nursing interventions

- Feeding, wound care and other skilled services when included in the plan
Personal Care Support
Hands-on assistance with activities of daily living when medically necessary and authorized.
- Bathing, dressing and grooming
- Mobility and transfer assistance
- Meal-related and other approved daily-living support
A parent or legally responsible caregiver may be able to provide approved personal care.
Georgia currently permits reimbursement of legally responsible individuals for medically necessary GAPP personal care services when program, authorization and provider-agency requirements are met.
This is a structured Medicaid option.
